The blank Pioneer Submission Form can be downloaded below. Once downloaded, your Pioneer Submission form can be opened and filled-in from your home computer. When finished, just print and signed. If you add information by hand after printing, please print legibly in block letters. A completed Pioneer submission is also provided to help give you as an example.

Special instructions for completing your Pioneer submission
Note: to have your ancestor be accepted as Pioneer, you must provide proof of their residence in Rock County, Wisconsin through solid research by providing reliable footnoted sources, such as census or courthouse records, church records, land or tax records.

- You must complete a Pioneer Submission Form for your ancestor, including your signature at the bottom agreeing to allow the Society to index, archive, compile and publish your Pioneer family along with others' submissions,
- You must include at least one photocopy of an original record that specifically demonstrates that the your Pioneer resided in Rock County in 1870 or earlier. In addition, we strongly encourage carefully documented sources for all key facts, including citations and photocopies of source documents, and
- In addition (not instead of the Pioneer Submission), submitters are encouraged to include pedigree charts and family group sheets that help document their pioneer and his or her family, and
- Finally, return your completed pioneer submission form by US Mail to:
